Comparing blood pressure measurement over a sleeved arm verses rolled up sleeve in adult patients in Emergency Departments Abstract Introduction Accurate Blood Pressure (BP) measurement is an increasingly important issue in nursing practical, the blood pressure identification when heart contraction and pump blood through artery to whole body the force generate pressure exerted by the blood upon the walls of the blood vessels and especially arteries, the systolic blood pressure during the contraction of heart , diastolic blood pressure during relax of heart, the blood pressure detected by the amount of blood pumped through your heart and the resistance of blood flow in your artery which means more blood pump and narrow of artery high blood pressure happen, for blood pressure measurement there is manual Sphygmomanometric and electronic device Oscillometric, each nurses taken blood pressure daily or hourly and there is many factor affect on blood pressure such as not resting 3-5 minutes ,crossed legs, position of arm, quality of device, emotional state, full bladder, talking, smoke and caffeine (Cader, Das & Kahhar, 2015). When these factors combined a significant different may appear. This paper will evaluate the comparison between blood pressure measurement over sleeved arm and rolling up sleeved in adult patients in Emergency Departments (EDs). Research Questions Is there any significant difference in systolic BP measurement over sleeve and roll up sleeve? Is there any significant difference in diastolic BP measurement over sleeve and roll up sleeve? What factors contribute to difference BP measurement over sleeve and roll up sleeve? Methodology / Design An experimental study will be conducted to measure blood pressure over sleeve arm and without sleeve (bare arm) in (EDs). Oscillometric BP device will be used for measurement in emergency department setting. For each participant BP will be taken 2 times using Oscillometric device over sleeved arm and rolled up sleeve. Prior to BP measurements, participants will be instructed to empty bladder if required, rest for 3 min in a sitting position, uncrossed leg while some demographic data about the participant will be collected once they agree to participate in the study. Participants and Approach The sample consists of 115 adult participant aged between 18 – 60 years including Male and Female patients who visited the Emergency Departments at two local hospitals. All measurements were taken with the assistance of two registered nurses who were trained on the procedure and documentation of data. No follow up was required and no identification data were collected. Inclusion Criteria of Participants Patients who attended the local ED in two public hospitals Age between 18 – 80 years old Acuity of illness is determined by Triage category of 3, 4 or 5 (indicates waiting time to receive emergency care) Exclusion Criteria Patients above 80 years and younger than 18 years Triage category of 1 or 2 Patients with Peripheral Arterial Disease (PAD) Patients with Chronic or Acute Renal Failure Agitated or aggressive behavior Patients with mental illness or substance abuse Ethical Considerations Ethical approval was granted from local bioethical committee (approval no ******).
